We also know that home is one of the most effective places to begin healing. Our home-based care model allows us to meet patients in familiar surroundings, helping to reduce triggers, build trust, and provide care without the added stress of hospitalization.
As the need for mental health services continues to rise, our response is rooted in innovation and community. We’ve built team-based programs designed to help individuals maintain autonomy, safety, and emotional stability-while staying connected to the people and places they love.

Our caring team evaluates each patient and creates a plan to help treat and navigate their mental health journey.

Additional Mental and Behavioral Health Programs
A team consisting of a mental health registered nurse, social worker, psychiatric nurse practitioner, Community Health Worker, Behavioral Health Occupational Therapist create a plan to treat patients 18 and older with the diagnosis of a severe mental and/or behavioral health illness and with demonstrated poor coping/maladaptive behaviors/frequent rehospitalization.
